New Enrollments - District Registration

Parents wishing to enroll their son or daughter in the Collinsville Community Unit School District may do so at the district's administration building:

TO ENROLL YOUR SON OR DAUGHTER, YOU WILL NEED TO BRING WITH YOU...

1. Certified Birth Certificate. The certificate issued by the hospital is NOT acceptable according to Illinois State Law. 
2. Social Security Number of the child you are enrolling.
3. Proof of residency - current monthly utility bill (electric, gas, water, cable), current Illinois public aid medical card, or closing papers on your house.  If you do not have any of these in your name, a residency check can be made to verify your residency.

   The following are NOT VALID for proof of residency:

   Receipt for having water turned on	Voter registration card
   Illinois driver's license	Library card
   Real estate tax bill	Mortgage payment book
   Employment letters	Fire arm ID card
   Gun registration card	Rent receipt
   Automobile registration	Cellular telephone bill

On or before the first day of school, your child must present a completed physical examination form with all immunizations listed as required by Collinsville Unit 10.  Your child will also be required to have a dental exam by May 15, 2006.
